You can try to remove your little tadpole parasite in Baldur's Gate 3 through eye surgery from Volo. But will it be successful?
In order to unlock this side quest, you have to save Volo from the Goblin Camp in Act 1.
If you decide to let Volo stay at your camp, after saving him from the Goblin Camp, he will devote his time to finding a cure for your little tadpole problem.
After a while, you can talk to him, and he will present his grand idea: eye surgery! Well, at least something that resembles eye surgery. It's more poking your eye with a needle while trying to get the parasite.

But is this actually a solution to your tadpole problem? No, sadly it isn't. But, the surgery still has its perks.
Baldur's Gate 3: Why You Should Still Consider Volo's Eye Surgery
Before choosing whether you want to get eye surgery done by Volo, you should be aware that this decision is irreversible. You will have multiple opportunities to back out of the procedure while Volo pokes you with a sewing needle, though. There are no consequences if you choose to refuse Volo's offer.
Be warned, your party members will not approve of you getting this surgery. But, their disapproval won't have any irreversible consequences. You can always schmooze up to them at a later point and restore your relationships.
If you decide to go through with the operation, Volo will eventually become frustrated, after missing the parasite multiple times with his needle, and turn to other measures. He'll put away his needle and grab an ice pick (is this man even board certified?) to remove your little tadpole. Spoiler, this attempt will also be unsuccessful and will result in you losing your eye instead.
On the plus side – Volo will insert a mechanical prosthetic that allows you to see invisible objects and creatures!
While your eye may not fit the aesthetic that you created for your character anymore, the surgery gives you quite the upgrade!
Once the surgery is done, Volo will leave your camp.
